All-on-6: full prosthetics with aggressive periodontitis and missing posterior teeth

The patient came to Allure Dental requesting restoration of smile function. But the clinicalpicture turned out to be significantly more complex than it initially appeared.

As a result of aggressive periodontitis, the front teeth had severe mobility and significant boneloss around the roots. In the posterior zones — missing chewing teeth. Chewing function waspractically lost, which substantially affected quality of life and overall health.

This is one of those cases where partial solutions simply do not work: preserving mobile teethwith such bone loss is not possible, and restoring the chewing zones without addressing thefront teeth would be pointless.

Solution

After thorough diagnostics, the optimal solution was proposed — total prosthetics with fixed constructions using the All-on-6 protocol. Six implants per jaw provide a greater support surface and better load distribution compared to All-on-4 — especially important with reduced bone density following periodontitis.

Treatment was planned in stages: restoration of chewing function, correct bite, and smile aesthetics — as a single comprehensive result, not three separate tasks.

Result

Full chewing function restored. Harmonious aesthetic appearance achieved. The result exceeded the patient's expectations.

This case is a strong illustration of why early periodontitis diagnostics and timely treatment matter more than any aesthetics. Periodontitis is not a cosmetic issue. Left untreated, it destroys bone tissue and ultimately leads to complete tooth loss. But even in complex cases, modern implantology makes it possible to fully restore both function and appearance.
